Abstract:Colleagues are currently covering multiple staffing gaps and each drugs trolley has a variety of drugs and a different layout which could result in delays to patients receiving their medication or receiving the wrong medication in error along with poor staff satisfaction. Following a meeting with the Deputy Director of Nursing- Medicine, it was requested that all wards within the division are planned in to have a review of the drugs trolleys on the ward area and encouraged to utilise 5S principles.

Critical Care Patient Diary (2022)

Abstract:
Following an initial trial of a basic patient diary, the concept was taken to the next level with further testing with colleagues and patients, to create an improved version which has become embedded into the culture of our ITUs. Key wins include a foreword from a well renowned children’s author (Michael Rosen), updated design with support from the Improvement Hub and our graphics team, and a training video alongside documented standard work. As a result, feedback has been very positive and is now well embedded into our daily work within both ITUs across SaTH.

Abstract:
In order to maintain the safety of our patients and help reduce the risk of falls, falls sensor mats were trialled on Ward 10 and 11. The mats have resulted in an overall reduction in falls of 55% on Ward 10 and 62% reduction in Ward 11 during the trial period.
